Routine visits with your doctor can ensure your diabetes is well managed. If a test reveals a problem, you can get early care to prevent it from getting worse.

Hemoglobin (HbA1c) Test

Get your HbA1c test to check how well your diabetes is being controlled. This is a blood test that measures your body’s average blood sugar over the last few months. You can earn up to $30 for completing the HbA1c test twice a year.

Dilated Eye Exam

Annual Flu Vaccine

Health Coaching

Iowa Total Care offers FREE health coaching through Iowa Total Care’s Healthy Solutions for Life program for our members. The program provides telephonic outreach, education, and support services.

  • $10 per year for Initial Intake Assessment.
  • $30 per year for Creation of Care Plan with Measurable Goals.
  • $50 per year for Successful Closure of Care Plan.
     

Take your medications as prescribed.

Your diabetes treatment plan may include insulin and/or oral diabetes medications to help keep your blood sugar normal. You may also need medication for other related health problems, like high blood pressure or high cholesterol.

Work closely with your doctor to take your medicines exactly as prescribed. It’s important to know the name, dose, and how-to for each of the medications you’re taking.

Don’t forget to get these other health checks and tests:

  • Blood pressure: Have your blood pressure checked at every office visit, or at least once a year. You should aim for a blood pressure reading of <less than 140/90 mm Hg>.
  • Kidney screening: Get a urine and blood test to check your kidney function and detect problems early.
  • Foot exam: Have your feet examined at every visit.
