Health Home

A "health home" is an approach to care that connects members with a team of professionals for the purpose of achieving healthier results related to chronic medical conditions, and it ensures all of the member's healthcare needs are met. There are two types: Chronic Condition Health Home (CCHH) and Integrated Health Home (IHH).
